Promoting Independent Living for Seniors in Leelanau County

Services

Directly and simply, with just one call to 231-256-0221, ShareCare Members arrange for any available  ShareCare services and programs. During regular office hours, ShareCare staff make arrangements with health care providers, experienced contractors or qualified volunteers to provide services you request.

In-Home Assessment

Each new Member receives an in-home ‘get acquainted’ visit by ShareCare’s registered nurse. She completes a general health assessment to ensure an understanding of each Member’s needs and wishes. Home safety is discussed, making certain that Members are safe in their homes.

Professional Health Care Coordination-at no additional charge

ShareCare's registered nurse, an experienced  health care coordinator and patient advocate, coordinates Members' health care services, including assisting with hospital discharge plans. She helps resolve overwhelming family situations and acts as an informative and reassuring contact to out-of-town families.

Friendly Visitors

Isolation, loneliness, and depression are major concerns influencing the well being of older adults. Being a companion and demonstrating interest in their well-being, ShareCare ‘friendly visitors’ play a vital role in helping Members maintain social interaction and a positive outlook.

Respite

Family members taking care of a loved one occasionally need time away from home to keep appointments, run errands, or just take a temporary ‘time out’ (respite) from the intensity of constant care-related duties. When appropriate, volunteer-provided respite services are most welcome in cases where special skills or medical knowledge are not necessary. Frequently the family caregiver can arrange for any special needs before leaving for a couple of hours.

Household Help

A list of trusted independent service providers, including caregivers, is available at no additional charge. These contractors who provide a variety of services, are selected, hired and paid directly by the Members who employ them.

Social Gatherings

ShareCare offers programs designed to bring Members together. Events include an annual meeting and volunteer recognition, a Royal Banquet honoring 90 year-old members, instructional forums and a craft fair displaying Members' artistic and hobby creations.

Access to Medical Equipment

Durable medical equipment is available for loan to Members at no cost. Typically available are wheelchairs, crutches, canes, walkers, bedside commodes, reacher-grabbers and bath benches.
