staff will make arrangements with health care providers, experienced contractors or qualified volunteers to provide services you request.

In-Home Assessment
Each new member receives a free, in-home ‘get
acquainted’ visit by ShareCare’s registered nurse. At that time, a
general health assessment is completed to ensure an understanding of
each member’s needs and wishes. Home safety is also discussed, making certain
that members are safe in their homes.
Professional Health Care Coordination-at no additional charge
Our registered nurse is an experienced case
manager, health care coordinator, advisor and patient advocate. From
coordinating health care services to assisting with hospital discharge
plans, her creative solutions frequently improve what might otherwise be
an overwhelming family situation. Our ShareCare nurse can also act as
informative and reassuring contact of out-of-town families.Household Help
A list of independent service providers, which
includes caregivers, is available at no additional charge. These
contractors, who provide a variety of services, are selected, hired and
paid directly by the members who employ them. Members pay only for the
services that they use. Before being added to our roster, caregivers are
interviewed and reference and background checks are performed. |

24 Hour Access to Medical Consultation
Whenever health concerns arise, ShareCare is there to help.
In conjunction with Munson Home Health Care, a registered nurse
is available by phone 24 hours a day, seven days a week to provide basic
medical advice to members or to help resolve a problem that shouldn't
wait.
Friendly Visitors
Isolation, loneliness, and depression are the
major concerns influencing the well being of more members than we realize.
It is for these people that ShareCare may make its most meaningful
contribution. By being their regular companions, giving special
attention to them on holidays and birthdays, and by demonstrating our
interest in their well-being, ShareCare ‘friendly visitors’ can play a
vital role in helping members maintain social interaction and a positive
outlook.
Respite
When family members are acting as caregivers,
those caretakers need to occasionally take time away from home. To avoid
burnout they must have time to attend to their own needs…to keep
appointments, run errands, or just take a temporary ‘time out’ (respite)
from the intensity of their constant care-related duties. When
appropriate, volunteer-provided respite services are most welcome in
cases where special skills or medical knowledge are not necessary.
Frequently the family caregiver can arrange for any special needs before
leaving for a couple of hours.
Neighborhood Gatherings
ShareCare offers programs designed to
bring members together in their neighborhoods. These are usually luncheons at which local professionals lead
informational discussions about important late-life issues.
The informality and camaraderie of the gatherings is appreciated
during Leelanau’s long winter months. Other social events include an
annual dinner, a volunteer luncheon, and a craft fair displaying
members' artistic and hobby creations.
Access to Medical Equipment
Durable medical equipment is available for loan
to members at no cost. Items typically available include wheelchairs, crutches,
canes, walkers, bedside commodes, reacher-grabbers and bath benches. |
